Report an Incident or Injury
Need to report an incident?
Fill in our Incident Report Form.

Need to report an Injury?
If you are injured while playing or training in sanctioned activities, it is important that you follow the steps below. Especially if you take any time out from the activity (even a few minutes to recover).
  • Make sure your coach knows. They will organise any first aid you may need, and they will let the association know about your injury by using the In-Game Injury Report .
  • Get checked out if you need. If there is any suspicion of concussion, don’t second guess this. Go to a GP or emergency department and get checked out.
    • All benches have a Concussion protocol form on display (CRT6) to help identify early signs of concussion.
  • Fill out the IHA Injury report. This lets us know more about your injury and any possible recovery time. It also helps the association track recurring injuries in our leagues.
  • If you need to make an insurance claim, you should now have all you need to be able to do this. Always access the form from the IHA insurance site to ensure you have the most current document.
    • You have 30 days from injury to submit your claim, which includes information completed by your treating physician.

The notices and QR Codes to access the various forms are located at the Ice Arena on the side of the IHSA Lockers (near Change Room 4) for easy access.
Always follow directions on the IHSA or IHA website for up-to-date information on making a claim.
